Work Hours/Shift:

  • Full-Time, Monday – Friday

You Will Be Responsible For:

  • Will hold the primary responsibility for connecting an integrated system of care across the employed medical group practices,

including:

a. Coordinating the transition to value-based care (managing populations)

b. Developing relationships to achieve results

c. Promoting standardization

d. Driving innovation by developing workflow / process mapping to design sustainable models

e. Creating process, structure, and care teams necessary to succeed in a value-based model

f. Creating workflow design and role responsibilities to optimize the care team

Supervisory Responsibilities:

a. May lead teams or committees to spearhead initiatives and projects.

b. Acts as an influencer to lead successful implementation of value-based models in all medical group divisions

  • Assesses practices for risk readiness and formulates recommendations for improvement

  • Obtain knowledge of all value-based contracts in which AdventHealth is positioned

  • Actively review data to identify gaps to performance giving guidance to optimize performance as needed

  • Develops plans of action that establish objectives, define strategies, and outline in detail the operational actions needed to meet the

  • objectives of success in value-based care

  • Communicate findings and facilitate Value Based Care Performance Reviews with medical group leaders and key stakeholders

  • Collaboration and integration with medical group leaders and other key stakeholders across the continuum of care to develop a

  • seamless system of care, achieving stated strategic objectives

  • Responsible for successful operational initiatives, and quality & safety adherence in all areas of scope of responsibility.

  • Ensures care team models are developed to meet the functions of patient care services and ensures appropriate level of staffing to

  • support organizational plans in responsible departments.

  • Define roles and responsibilities of care team members and develop performance standards that meet needs of patients and

  • community.

  • Assures compliance with policies, procedures, standards of care, and standards of practice in all areas where patient care is delivered.

  • Meets identified objectives and established deadlines on a consistent basis.

  • Other duties as assigned by the Physician Enterprise executive team.

Qualifications

What You Will Need:

  • Bachelor's degree in a clinical role (RN, RT, PA, etc.) and minimum of ten (10) years health care experience

  • Completion of one professional certification within one year and maintenance of certification thereafter (Lean Bronze Certification, CMPE, Lean/Six Sigma Certified Green or Black Belt)

KNOWLEDGE AND SKILLS REQUIRED:

  • Experience in Value Baes Care Models or Population Health Management

  • Strong knowledge of patient safety and patient care management principles

  • Familiarity with physician practice transformation in the multi-specialty setting, supported by project management and quality improvement principles

  • Knowledge of quality reporting and physician practice regulatory programs

  • Knowledge in Healthcare Operations, Information Technology, Revenue Cycle, Practice Operations, Clinical Operations, Nursing Workflow, Physician Workflow, Medical Information

  • Proficiency in identifying, defining, analyzing, and displaying metrics and goals

  • Expertise in process improvement methodologies and process mapping

  • Demonstrated leadership or prominent participation in practice management/EHR implementation and/or ongoing support and optimization programs

  • Management and leadership experience building and directing large, cross-functional work teams

  • Effective communication skills with all stakeholders, e.g., executive leadership, physicians, managers, and staff

  • Microsoft Office suite experience with advanced PowerPoint and Excel skills

Job Summary:

The Director of Value Based Care has the responsibility for planning, organizing, implementing, and evaluating value-based practice operations into a seamless system of care across the employed providers' medical group practices. This work aligns and directly connects with the strategic priorities of AdventHealth and the Managed Risk Division. The Director of Value Based Care will report directly to the Executive Director and will lead activities to maximize risk readiness and performance in risk-based contracts, drive whole person population health efforts, and coordinate care across the continuum to decrease the total cost of care.
